CHECKLIST FOR PASSPORT APPLICATION REQUIREMENTS
REPLACEMENT OF LOST OR MUTILATED PASSPORT/ASSUMED NAME/
PASSPORT WITH DISCREPANCY (Name / Date of Birth / Place of Birth)
  1. Personal appearance of applicant;
  2. Duly accomplished application form;
  3. Original and photocopy of authenticated NSO Birth Certificate;
  4. If married, original and photocopy of authenticated NSO Marriage Certificate or Report of Marriage;
  5. Original and photocopy of authenticated Form 137 (Elementary/High School), Yearbook or Transcript of Records;
  6. If the lost passport contains a valid visa, submit Police Report of Loss (ishitsu shomei) from the nearest police station; if the loss was due to a fire or accident, submit a Certificate of Affliction (risai shomeisho) from the fire department. Note: The Consular Officer reserves the right to require additional proof or documents, pursuant to the Philippine Passport Law (R.A. 8239) for the purpose of verifying the identity, citizenship and personal circumstances of the applicant.
  7. Affidavit of Loss or Destruction of Passport or Pinanumpaang Salaysay
  8. Letter Pack 500 envelope; and
  9. Fees for lost passport:
